Combined Assessment Program Review of the Minneapolis VA Health Care System Minneapolis, Minnesota\n\r (9/13/2011)

The purpose of the review was to evaluate selected operations, focusing on patient care administration and quality management (QM). During the review, OIG provided crime awareness briefings to 202 employees. This review focused on eight operational activities. The facility complied with selected standards in the following four activities: (1) enteral nutrition safety, (2) management of workplace violence, (3) medication management, and (4) registered nurse competencies. The facility’s reported accomplishments were having the lowest 30-day mortality rates for acute myocardial infarction and heart failure among Minnesota hospitals and a facility physician who received a Veterans Health Administration ethics award. OIG made recommendations for improvement in the following four activities: (1) environment of care, (2) QM, (3) coordination of care, and (4) physician credentialing and privileging.
